RT Health Top and Smart Extras

A benefit for gym membership is payable where the gym is registered with AUSactive and a letter of recommendation from a Medical Practitioner, Medical Specialist, Podiatrist, Dietitian, Exercise Physiologist, Physiotherapist, Osteopath, Chiropractor, Occupational Therapist or Psychologist stating the medical reason the member needs to attend the gym and supplied together with a gym membership receipt. The letter will be valid for 12 months, after which a new letter will be requested before further claims can be paid.

All AUSactive business membership invoices, must display:

Details of service used (e.g. OsteoStrong bone denisty program).

• Client name
• Business name
• ABN
• Australian address of the facility

Your RT Health provider number is your AUSactive business membership number and must be displayed.
